Beyond the Count: Understanding Modern Concussion Protocol

In the world of combat sports, head trauma is a serious risk. The old "shake it off" mentality is dangerous and outdated. Modern sports medicine research has shown us that concussions are far more serious than previously understood, and proper management is essential for both short-term recovery and long-term brain health.

Understanding the signs of a concussion and adhering to a strict recovery protocol is essential for your long-term health and career.

What Is a Concussion?

A concussion is a mild traumatic brain injury (mTBI) caused by a bump, blow, or jolt to the head—or by a hit to the body that causes the head to move rapidly back and forth. This sudden movement causes the brain to bounce around or twist inside the skull, creating chemical changes and sometimes damaging brain cells.

Key Signs to Watch For Immediately After Impact

Physical symptoms:

  • Headache or a feeling of "pressure" in the head
  • Nausea or vomiting
  • Dizziness or balance problems
  • Blurred or double vision
  • Sensitivity to light or noise
  • Feeling sluggish, hazy, or foggy

Cognitive symptoms:

  • Confusion or feeling as if in a fog
  • Difficulty concentrating or remembering
  • Answering questions slowly
  • Not knowing the date, time, or location

Emotional symptoms:

  • Irritability or mood changes
  • Sadness or anxiety
  • "Just not feeling right"

Red flags requiring emergency care:

  • Loss of consciousness (even briefly)
  • One pupil larger than the other
  • Worsening headache that won't go away
  • Seizures
  • Slurred speech
  • Repeated vomiting

Important: If you experience any of these symptoms, your training session must end immediately. Never return to sparring or competition on the same day you suspect a concussion.

The Modern Return-to-Play Protocol

The current standard is a graduated return-to-play protocol with at least 24-48 hours between each step. If symptoms return at any stage, you must go back to the previous step.

Cognitive Rest Matters Too

Proper recovery involves both physical and cognitive rest. In the first 24-48 hours, this means:

  • Limiting screen time (phones, computers, TV)
  • Avoiding mentally demanding tasks
  • Getting plenty of sleep
  • Staying in quiet, low-light environments if symptoms worsen with light/noise

When to Seek Professional Care

While most concussions resolve within 7-14 days, some individuals experience prolonged symptoms (post-concussion syndrome). Seek evaluation if:

  • Symptoms persist beyond 2 weeks
  • Symptoms are worsening rather than improving
  • You've had multiple concussions in your career
