• PIP (Personal Injury Protection): Florida law requires treatment to begin within 14 days of an accident for PIP benefits to apply. We can assist with PIP claims, but coverage amounts and eligibility are determined by your insurance policy and applicable law
• Insurance verification: We will verify your insurance coverage prior to treatment when possible, but final coverage determinations are made by your insurance company
• Attorney liens: We accept attorney liens for qualified personal injury cases. Terms are established between the patient, attorney, and our office
• No guarantee of coverage: Listing an insurance provider on our Site does not guarantee that your specific plan will cover our services. You are ultimately responsible for any charges not covered by your insurance

To the fullest extent permitted by applicable law, Windmill Health Center, Dr. Eliot Corvin, and their officers, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of data, loss of profits, or business interruption, arising out of or related to your use of or inability to use the Site.
